Undergraduate

1 .个人普通护照照片页。 如在中国，请同时提供签证页或居留许可页。

2 .高中毕业证书。 如申请人为在校学生，须提交本人就读学校出具的预毕业证明，录取后须补交高中毕业证书。中英文以外文本须附经公证的中文或英文翻译件。

3 .高中学习成绩单。 中英文以外文本须附经公证的中文或英文翻译件。

4 .学习计划。

5 . 汉语授课专业申请者提供HSK4级 或以上成绩报告。

6 .银行存款证明。 存款证明金额要能涵盖在校期间的学费、宿舍费和生活费。以申请人本人、父母或其他监护人的名义出具。

7. 无犯罪记录证明。

8 .《外国人体格检查表》。 申请人应严格按照《外国人体格检查表》要求项目进行检查。缺项、未贴本人照片或照片上未盖骑缝章、无医师和医院签字盖章的《外国人体格检查表》无效。检查结果有效期为6个月。

9.其他辅助证明材料（如有请提供，例如获奖证书、培训证书等）。

10.未满18周岁的申请人，须提交《在华监护人公证书》。

ThePhysical Examination Form for Foreignerswith missing items, no photo of myself or no seal of riding and sewing on the photo, and no signature and seal of doctors and hospitals is invalid. The inspection results are valid for 6 months.

7. Applicants under the age of 18 must submit the Notarial Certificate of Guardian in China.
