（1）《2019年上戏一带一路艺术文化人才培优项目报名表》；
（2）护照复印件，或电子扫描件（护照复印件需要贴有照片的一页）；
（3）个人简历；
（4）在读证明或毕业证明书
（5）一张报名照和一张生活照
3、相关费用：
奖学金学员免报名费、学费、住宿费、活动费、教材费、及来华人员综合保险费；餐费、往来国际、国内旅费自理。
4、报名程序及注意事项：
（1）报名表需网上下载后填写。请根据要求用中文或英文如实填写。

（2）所有报名材料通过邮件方式递交。
（3）学院审核确认申请者的报名材料后，将向申请人发放《录取通知书》和《外国留学人员来华签证申请表》，申请人务必须持学生签证（X1或者X2）入境。录取通知书和签证申请表将通过邮寄的形式发送给申请人，请在填写报名表时务必留下可供联系的通信地址和电话。
（4）申请者要在录取通知书规定的报到日期来校报到。因故不能按期报到者，必须事先征得留学生办公室同意，否则将视为自动放弃。
（5）提交的申请材料不予退还。
