1.国籍、健康状况
Nationality&Health Condition
外国籍公民，持有外国护照，身体健康；
Foreigncitizens, have foreign passport, in good physical and
mental health.
2.年龄、学术背景
Age& Academic background
硕士研究生:硕士研究生申请者年龄须在40周岁以下,持有有效本科学位学历证书。
Applicants who apply for the master’s degree must be under
40years old and have the valid bachelor’s degreecertificate.
博士研究生申请者年龄须在45周岁以下（截止时间），持有硕士学位学历证书。
Applicantswho apply for the Ph.D. degree must be under 45
years old and have the validmaster’s degree certificate.
注：部分优秀学生年龄可适当放宽。
Notice: Theage of some outstanding applicants can be
enlarged appropriately.
3.申请人应有较强的科学研究水平。
Applicants should have strong capacity in scientific research.
4.语言要求 Language requirements
汉语水平需要在HSK四级或以上； HSKK证书（中级及以上）；中文为母语者。
HSK level ≥4 or HSKK certificate (Intermediate or above)
or nativeChinese speakers.

四、申请材料Application Documents
申请人须在长春中医药大学国际学生在线服务系统（https://ccucm.***/member/login.do）报名并提交以下申请材料：
Applicants should complete and submit the following
documents inthe online application system:
Application form (auto-generated after the online registration)
2.《外国人体格检查表》复印件（原件自行保存）
申请人应严格按照《外国人体格检查表》中要求的项目进行检查。缺项、未贴有本人照片或照片上未盖骑缝章、无医师签字或医院盖章的《外国人体格检查表》无效。鉴于检查结果有效期为6个月，请申请人据此确定体检时间并携带原件到校报到。
A copy of physical examination form (the original shall be
kept byyourself). The applicant shall carry out the
examination in strict accordancewith the items required in
the foreigner's physical examination form. Theforeigner
physical examination form without the signature and seal of
thedoctor or the hospital is invalid. Since the examination
result is valid for 6 months, please determine thetime for the
examination according to it. Please bring the original to the
school.
3.高学历证明
（中英文以外文本须附经公证的中文译本，应届毕业生须提交预毕业证明，被录取后需补交高学历复印件）
Highestschool diploma (in Chinese/English or notarized in
Chinese/English, Freshgraduates must submit the
pre-graduation certificate and a copy of the highest diploma);
4.成绩单（中英文以外文本须附经公证的中文译本）
Academictranscripts (in Chinese/English or notarized in
Chinese/English );
5.个人陈述
（个人陈述必须用中文或英文书写；内容包括：自我介绍、学习经历及来华学习目的；字数不少于800字。）
Personal statement (Self-introduction,studying experiences
and a study plan should be includedin Chinese
language/English language, no less than 800 words);
6.汉语水平能力证明
（申请人汉语水平须达到HSK4级，HSK未达4级者需在我校补习一年汉语。）

Chinese Requirement: HSK≥level 4 Applicants whose
HSK ≤ level 4 should take extraclasses in the school;
7.有效普通护照复印件
A copy of a valid passport

（4）申请者资格Eligibility
①非中国籍公民，身体健康；
Non-Chinese citizens, ingood physical and mental health;
②截至2020年9月1日，来华攻读硕士学位者，须具有学士学位，年龄不超过35周岁；来华攻读博士学位者，须具有硕士学位，年龄不超过40周岁；
Up to 1 September, 2020, applicantswho apply for the
master’s scholarship should get their bachelor’s degree and
be under 35 years old; Applicants who apply for the Ph.D.
scholarship should gettheir master’s degree and be under 40
years old;
③成绩优秀，科研能力突出，汉语HSK4级或以上;
Applicants should have goodacademic marks and have
outstanding capacity in scientific research; HSK≥level 4;
④非其他奖学金获得者。
Without winning other scholarships.
（5）申请材料ApplicationDocuments Required
申请人必须如实填写和提交以下申请材料：
Applicants should complete and submit the following
application documents:
①《中国政府奖学金申请表》（用中文或者英文填写）；
Applicationform for Chinese Government Scholarship
(in Chinese/English).
②《外国人体格检查表》复印件（原件自行保存）；
Acopy of foreigner's physical examination form
(the original should be kept by yourself).
③经过公证的高学历证明（中英文以外文本须附经公证的中文或英文译本，应届毕业生须提交预毕业证明）；
Notarizedhighest diploma(in Chinese/English or notarized
in Chinese/English. Freshgraduates must submit the
pre-graduation certificate).
④本科或硕士阶段全部课程的成绩单原件（中英文以外文本须附经公证的中文或英文译本）；
Originalacademic transcripts of all master's degree or PhD
courses. (Documents in languages other than Chinese or
English must be attached with notarized Chineseor English
translations)
⑤两名教授或副教授的推荐信（中文或英文）；
Two recommendation letters from professors or associate
professors(inChinese/English).
⑥来华学习或研究计划，申请人必须提交用中文或英文书写来华学习或研究计划(硕士不少于800字，博士不少于1500字）；
A study plan in English or Chinese(no less than 800 words
for master'sdegree; no less than 1500 words for PhD).
⑦研究成果，已发表的论文、获奖证书等能够证明自己学业成就和研究能力的资料；
Notarized Copies of research achievements.
⑧汉语或英语水平能力证明（如HSK证书、TOEFL、IELTS成绩证明）；
Certificate of English/Chinese proficiency.
⑨有效普通护照复印件。
A copy of valid passport.
注：1.无论录取与否，以上材料恕不退还；2.材料不全者，不予受理；3.1～9项提交申请材料均须一式两份。

（6）申请程序How to Apply
第1步：登陆下面网站，进行网上报名，长春中医药大学代码为：10199；
Step1: Visit the website below and register online. Code of
Changchun University of Chinese Medicine :10199.
留学基金委来华留学网上报名系统：
Registrationsystem of China Scholarship
长春中医药大学国际教育学院网上报名系统（必须进入系统报名否则材料不予受理）：
Applicationsystem of the College of International Education
of Changchun University of Chinese medicine (must enter the
system or the documents will not be accepted)
第2步：完成留学基金委来华留学网上报名后，打印系统自动生成的《中国政府奖学金申请表》，按照“申请材料”清单的要求准备相应材料；
Step 2: After completing the online application of the China
Scholarship Council, print the application form for the
Chinese governmentscholarship which the system
automatically generates, and prepare related documents
according to the requirements of the list of “Application
Documents”.
第3步：邮寄所有申请材料至长春中医药大学国际学生招生办公室。
Step3: Mail all application documents to the international
student admission office of Changchun University of
Chinese Medicine.
（7）录取和入学Admission and Entrance
第一步：审核申请材料，通过材料审核后，招生办公室将组织相关专家进行网络或现场面试。终录取结果将在7月15日左右公布（具体时间以国家留学基金管理委员会通知为准）；
Step1: Applicants will be selected according to the
application documents andinter view. The first selection
result will be seen in late April. The finalresult will be seen
on 15 July or so (see the China Scholarship Council's notice).
第二步：入学时间为每年9月15日之前，具体时间以《入学通知书》为准。被录取的学生须严格按照入学通知书规定的时间到长春中医药大学国际教育学院办理入学手续。超过规定时间未办理入学手续者，将被取消入学资格。
Step 2:The registration deadline is15 September 2020
(See the admission notice). If you fail to register withinthe
time limit and without the permission of the registration
office of the College of International Education, you will be forfeited admission to thisuniversity.
2.吉林省政府奖学金JILIN Government Scholarship
（1）报名截止日期：2020年5月30日。
